How to Request Your Health Care Records

As a consumer, you have the right to access your health care records. To request copies of your records from our practice, please follow the steps below:
Submit a Written Request:
You may request your health care records by submitting a written request. You can send your request by email, mail, or deliver it in person to our office. Please include your full name, date of birth, contact information, and specify the records you are requesting.
Verification and Processing:
For your privacy and security, we may ask you to verify your identity before processing your request. Once your request is received, we will respond within 15 days as required by Texas law.

Fees:
There may be a reasonable fee for copying and delivering your records, as allowed by law. There is a $15 charge for electronic records. The fee for paper records is $0.50 per page, plus any shipping charges. We will inform you of any applicable fees prior to processing your request.
